DSR Policies/Procedures

加兹登州 Community College has adopted an internal grievance procedure providing for prompt 和 equitable resolution of complaints alleging any action prohibited by the U. Steps to File a Complaint

  1. An ADA Complaint Form must be filed, 以书面形式, 在涉嫌违规的十(10)个工作日内向ADA协调员提交. 它必须包含提交人的姓名和地址, 它必须简要描述涉嫌违反规定的行为.

  3. A written determination as to the validity of the complaint 和 a description of the resolution shall be issued by the ADA Coordinator 和 a copy forwarded to the complainant no later than ten (10) business days after the completion of the investigation, via certified mail, personal delivery or student email.
  4. 如果解决计划未能使投诉人得到满意的解决, the complainant may choose to pursue a formal appeal through the DSR office by requesting that the evidence be presented to the Disability Services Committee. 学生 may need to seek course substitutions on the basis of a documented disability that inhibits the ability to learn certain subjects. 虽然辅导和住宿可能会帮助一些学生在这些课程中取得成功, 其他人可能无法成功,即使使用学术调整和住宿. 在这种情况下,学生可以提交课程替换请求.

2. Student must complete the Course Substitution Request Form or submit a letter/statement to the ADA Coordinator specifically requesting the substitution as an accommodation for the educational functional limitations caused by the documented disability. This letter/statement should include:

  • The student’s name, address, G number, 和 major
  • The name of the course where the substitution is requested

  • A rationale for the substitution request

3. The ADA Coordinator, in coordination with the appropriate division chair, will review the student’s letter/statement, disability documentation 和 transcripts.

  • If the course in question is considered to be an essential part of the student’s program or a requirement for certification or licensure, a substitution can不 be granted.

4. The ADA Coordinator will submit the recommendation of the division chair to the appropriate Dean who will either approve or reject the request for substitution.

The decision of the Disability Service Committee shall constitute the final decision with respect to the student’s request; there shall be no further appeal.

Emergency Procedures:

学生 with documented disabilities should request reasonable accommodations from the Residence Hall Director before moving into the Residence Hall. The ADA defines a service animal as a dog that has been individually trained to do work or perform tasks for the benefit of an individual with a disability including a physical, 感觉, 精神病学, intellectual or other mental disability. 服务性动物的工作或任务必须与个人的残疾直接相关. Tasks may include, but 是 不 limited to, guiding individuals with impaired vision, 提醒听力受损的人注意入侵者或声音, pulling a wheelchair, or retrieving dropped items.

Restriction of the 服务的动物:

Under the ADA's regulations for 服务的动物, 服务的动物 have a right to be anywhere that a human is, unless an entity can demonstrate that there would be either a direct threat or fundamental alteration in the program or activity with the presence of the service animal. 也, 服务型动物也被限制进入实验室,除非他们有适当的个人防护设备.

Conflicting Disabilities:

Individuals with medical issues that may be impacted by the presence of 服务的动物 should communicate with Disability Services 和 资源 if they have a concern about exposure to a service animal. 个人将被要求提供证明残疾和需要住宿的文件. DSR will facilitate a process to resolve the conflict that considers the disability-related needs/accommodations of all persons involved.

PROCEDURES REGARDING SERVICE ANIMALS

CLARIFICATIONS REGARDING SERVICE ANIMALS

TYPES OF SERVICE DOGS:
